Where to Inject Mounjaro: Step-by-Step Guide to Sites and Technique

30 / Apr

Starting a new injectable medication can feel daunting, particularly if you have never self-injected before. One of the most common questions we receive at our weight loss clinic in Bristol is where to inject Mounjaro and how to do it correctly. The good news is that the Mounjaro auto-injector pen is designed to be straightforward and relatively painless when used properly. This guide walks you through everything you need to know, from the approved injection sites to step-by-step technique and tips for making each injection as comfortable as possible. If you have any concerns at all, contact our team for personalised support before your next dose.

The Three Approved Mounjaro Injection Sites

Mounjaro (tirzepatide) is administered as a subcutaneous injection, meaning it is injected into the fatty tissue just beneath the skin rather than into a muscle or vein. There are three approved sites on the body where this can be done safely and effectively.

Injection SiteLocationBest ForNotes
AbdomenAt least 5cm from the navelMost accessible site for self-injectionAvoid the area directly around the belly button
ThighFront or outer upper thighEasy to see and reachUse the middle third of the thigh for comfort
Upper armOuter area of the upper armUseful when rotating away from the abdomen or thighMay require assistance; harder to self-administer

All three sites are clinically approved and equally effective for absorption. The most important principle is to rotate between sites and within each site on a weekly basis to prevent skin changes at the injection point.

Before using Mounjaro, make sure you understand the basics of the drug with our introductory blog on Mounjaro here.

How to Inject Mounjaro: Step-by-Step

Following the correct technique helps ensure the medication is absorbed properly and reduces the risk of discomfort or skin reactions.

① Before You Begin
  1. Check the pen: Inspect the Mounjaro pen to confirm it has not expired and that the liquid inside is clear or slightly yellow with no visible particles. Do not use the pen if the liquid looks cloudy or discoloured.
  2. Allow it to warm up: Remove the pen from the fridge and leave it at room temperature for 30 minutes before injecting. A cold injection is more likely to cause discomfort.
  3. Wash your hands: Wash thoroughly with soap and water for at least 20 seconds and dry completely before handling the pen.
  4. Prepare the injection area: Clean the chosen site with an alcohol wipe and allow it to dry fully before injecting. Injecting into damp skin can increase stinging.
② Administering the Injection
  1. Remove the base cap: Pull off the grey base cap from the pen. Do not remove the clear cap until you are ready to inject.
  2. Remove the clear cap: Pull the clear cap straight off just before use. Do not twist it. You will see the needle is now exposed.
  3. Position the pen: Place the flat end of the pen firmly against your skin at a 90-degree angle. You do not need to pinch the skin unless you have very little subcutaneous fat at the chosen site.
  4. Press and hold: Press the purple injection button firmly with your thumb. You will hear a click indicating the injection has started. Hold the pen in place for 10 seconds to ensure the full dose is delivered.
  5. Confirm delivery:  Check that the injection window confirms the full dose has been delivered, as described in the instructions provided with your pen.
  6. Remove the pen: Lift the pen away from the skin. A small amount of blood or a red mark at the site is normal and not a cause for concern.
③ After the Injection
  1. Do not rub the site: Rubbing the injection area after removing the pen can cause irritation. Simply press gently with a clean cotton pad if needed.
  2. Dispose of the pen safely: Place the used pen in a sharps bin immediately. Do not recap the needle or dispose of it in household waste. Your pharmacy can advise on sharps disposal in Bristol.
  3. Record your injection: Note the date, dose, and site used. This helps you rotate correctly and keeps a clear log of your treatment.

Why Rotating Injection Sites Matters

Repeatedly injecting into the same spot can cause a condition called lipohypertrophy, where fatty lumps develop under the skin. These lumps can affect how well the medication is absorbed, potentially reducing its effectiveness without any obvious sign that something is wrong.

To rotate correctly:

  • -> Move to a different site each week (abdomen one week, thigh the next, for example)
  • -> Within each site, vary the exact location by at least 1–2cm from the previous injection point
  • -> Avoid areas with visible lumps, bruising, scarring, or broken skin
  • -> Avoid injecting into moles, stretch marks, or tattoos where possible

According to NHS guidance on subcutaneous injections, rotating injection sites consistently is one of the most important factors in maintaining reliable absorption of injectable medications.

Tips for a More Comfortable Injection

  • -> Always allow the pen to reach room temperature before use
  • -> Ensure the skin is fully dry after cleaning with an alcohol wipe
  • -> Breathe out slowly as you press the button to help you stay relaxed
  • -> If you notice persistent redness, swelling, or itching at the site, mention it at your next consultation
  • -> If injections are consistently painful, speak to your clinician about technique or site preference

Our Bristol weight management services include full injection training and ongoing clinical support, so you never have to figure this out alone. Whether you are on your first dose or several months in, our team is available to help.

What to Do If You Make a Mistake

Mistakes with injectable medication are more common than people realise, and most are easily resolved.

  • -> Injected into the wrong site: The medication will still absorb. Simply make a note and rotate correctly next time.
  • -> Not sure if the full dose was delivered: Do not administer a second dose. Check with your prescriber before your next scheduled injection.
  • -> Missed your injection day: If it has been four days or fewer (within 96 hours) since your scheduled dose, administer it as soon as you remember. If more than four days have passed, skip that dose and resume your regular weekly schedule.
  • -> Pen feels blocked or does not click: Do not attempt to force the injection. Contact your pharmacy for a replacement pen.

Frequently Asked Questions

Does it matter which site I use for Mounjaro?

No. All three approved sites produce equivalent absorption. The most important thing is that you rotate between them consistently and choose a site where you feel comfortable self-injecting.

Can I inject Mounjaro into my muscle by mistake?

The Mounjaro pen is designed for subcutaneous injection, and the needle length is calibrated for this. Accidental intramuscular injection is unlikely with correct technique. If you have very little subcutaneous fat, speak to your clinician about whether pinching the skin is advisable at your chosen site.

How do I store Mounjaro correctly?

Store unused pens in the fridge between 2°C and 8°C. Do not freeze them. Once removed from the fridge, a pen can be kept at room temperature (below 30°C) for up to 21 days. Always check the expiry date before use.

Is it normal for the injection site to be red or itchy afterwards?

Mild redness, itching, or a small raised bump at the injection site is common and usually resolves within a few hours. If the reaction is severe, spreads significantly, or does not resolve within 24 hours, contact your prescriber.

Can someone else inject Mounjaro for me?

Yes. A carer or healthcare professional can administer the injection if you are unable to do so yourself. The technique and approved sites remain the same. All three sites are accessible by a second person, including the upper arm, which can be difficult to self-administer.
